Primary Care Practitioner (Physician, DO, FNP, or PA)
Job Summary
Our Primary Care Practitioners provide high quality, comprehensive medical care and treatment to a diverse safety-net patient population in a patient-centered, team-based model with an emphasis on the prevention of disease, promotion of health, and management of chronic disease. The clinical team includes certified medical assistants, RN care coordinators, clinical pharmacists, health navigators, and behavioral health consultants.
To allow the time to deliver high quality care to patients with complex needs, our practitioners manage panels between 700 and 1,200 patients and see about 14 patients per day. After-hours call is shared among the full primary care group and supported by a nurse triage line that manages the vast majority of after-hours requests. Our providers have the option of teaching medical residents from several organizations as well as medical students from the local medical school.
Community Health Centers of Benton and Linn Counties (CHC)
The CHC opened as a Federally Qualified Health Center (FQHC) in 2004. It is the medical home for over 9,000 patients across six clinics and two counties. Our clinics are located in (click on the the link to learn more about each city):
The CHC provides physical, behavioral, dental care, and medical treatment for substance use disorders. The CHC works closely with Benton County Public Health, County Mental Health programs, and school districts at our two school-based sites.
The CHC utilizes the clinical team to support patients in accessing health. We are committed to improving health outcomes through a medical home model with a strong focus on prevention and disease management. The CHC sites are all Patient Center Primary Care Home (PCPCH) certified at a high level. Our organization serves communities with Health Professional Shortage Area (HPSA) ratings between 10 and 13 (https://data.hrsa.gov/tools/shortage-area/hpsa-find) and qualifies for National Health Scholars, National Health Service Corps and other Loan Repayment Programs.
- MD/DO annual salary range of $162,000 - $198,000.
- PA annual salary range of $95,281 - $129,568.
- FNP annual salary range of $93,597 - $135,364.

- These are FLSA exempt positions, not eligible for overtime.

Minimum Qualifications
- MD/DO/PA: unrestricted Oregon Medical License (license to practice in the State of Oregon or licensed in a different state with the ability to obtain an Oregon license).
- FNP: unrestricted Oregon Nursing License(license to practice in the State of Oregon or licensed in a different state with the ability to obtain an Oregon license).
- Valid Oregon driver's license.
- Physician providers are board eligible/certified in a primary care specialty (FP or IM).
- Nurse Practitioners holds FNP specialty certification.
- Hiring is contingent upon the successful completion of a background check.
- Benton County requires all Health Services and Clinic employees to complete their primary COVID-19 vaccination series or have an approved exception to the requirement due to a medical condition or sincerely held religious belief. Successful candidates for this position must submit vaccination documentation or be approved for an exception prior to their first day of employment. Failure to provide proof of full documentation or receipt of an approved exception will lead to withdrawal of the job offer.
